What is Care Coordination?
Care Coordination is a holistic, comprehensive, multidisciplinary approach to the planning and delivery of care and services to meet the needs of individuals with disability or addiction issues.
This approach incorporates assessment, identification of needs, care and service plan development, advocacy, negotiation, coordination and provision of necessary services, and referral linkages.
