What is Care Coordination?

Care Coordination is a holistic, comprehensive, multidisciplinary approach to the planning and delivery of care and services to meet the needs of individuals with disability or addiction issues.


This approach incorporates assessment, identification of needs, care and service plan development, advocacy, negotiation, coordination and provision of necessary services, and referral linkages.

Five Stages of Service

  • 1. Screening and Referral

    The first call you make to our office helps us determine your needs and how our services may help.

  • 2. Assessment

    We will ask you more about your present situation and background to find out exactly what we can do for you.

  • 3. Recovery Planning

    Our care coordinator will develop a plan based on your goals using input from you and your doctors.

  • 4. Care Coordination

    Our care coordinator will provide you with support as you work to accomplish the goals of your recovery plan.

  • 5. Ongoing Care

    Once you complete your recovery plan, ongoing support will be available as you need it.
